In a released investigation, the Joint Task Force-Red Hill states the Navy’s “quality assurance process did not identify and remedy the improperly installed air vacuum valve in April 2022 that led to the inadvertent AFFF release.”

Earlier this year, the military opened the Red Hill Clinic to provide treatment to those facing long-term health effects from the November 2021 jet fuel water crisis. It has seen about 130 patients since it opened in January, and about 75 have been referred to specialty care, as reported by the Defense Health Agency. HPR's Sabrina Bodon has more.

The Defense Department says it will permanently shut down the Navy’s massive fuel tank facility in Hawaiʻi that leaked petroleum into Pearl Harbor’s tap water, and will remove all the fuel. Defense Secretary Lloyd Austin made the decision Monday based on a new Pentagon assessment. The announcement was praised by Hawaiʻi's congressional delegation and local leaders.

A Hawaiʻi Department of Health official ruled the Navy must comply with a state order requiring it to remove millions of gallons of fuel from the Red Hill storage facility. Navy Secretary Carlos Del Toro announced Monday that the Navy has suspended the use of the World War II-era Red Hill fuel tank facility that sits above one of Honolulu’s most important aquifers and water sources. Del Toro apologized to everyone who has been affected by what he called "this horrible, horrible tragedy.”
