The Hawai?i Department of Public Safety has identified a staff cook supervisor at an O?ahu prison found dead Monday as 37-year-old Darwin Fernandez.
The department said Fernandez was found unresponsive at about 3:10 p.m. He was provided medical assistance but was confirmed dead shortly after.
The coroner will determine the cause of death. But Honolulu police, attorney general's office and public safety's internal affairs are investigating.
"We are fully cooperating with our law enforcement partners as they investigate the death and acknowledge that they need to be able to do their jobs without speculation from us on the how's and why's surrounding this tragic situation," said Public Safety Director Nolan Espinda in a news release.
Ferenandez was found unconscious in a staff bathroom at the Halawa Medium Security Facility. Officials say the bathroom is for staff use only.
The death comes two weeks after a controversial Senate confirmation of Espinda to continue to head the Department of Public Safety, which has come under fire amid numerous internal and external investigations.
